Inquisit.org Privacy Policy and Your Privacy Rights
This Privacy Policy is effective on March 1, 2010.
General
This Privacy Policy describes the practices that Health Industry Technology, LLC d/b/a Inquisit (referred to hereinafter as "Inquisit" or "we") follows with respect to the collection and use of information submitted on inquisit.org (the "Site"). This Privacy Policy applies only to information we collect from this Site, and does not apply to any other website or business activity of Inquisit. By using our Site, you consent to the collection and use of this information by us. We reserve the right to revise this policy at any time. When we do, we will give you notice of the revised Privacy Policy by posting the revised Privacy Policy on the Site and by revising the effective date at the top of this Privacy Policy. You are responsible for regularly reviewing the current privacy policy.
Information We Collect
We collect "personally identifiable information" and "automatic information." "Personally identifiable information" is information that tells us who you are, such as your first and last name, organization, credentials, profession, mailing address, e-mail address, phone number, fax number, credit card information and/or other information used to verify your identity when you retrieve your password. We may collect personally identifiable information when you (a) register with our Site and/or for any educational activities, (b) purchase audio conferences or other materials or services from us, (c) complete online surveys, or (d) if you voluntarily choose to provide it to us (e.g., if you send an e-mail to us). You can choose not to provide personally identifiable information to us. However, if you do not provide such information, you will not be able to (a) register with the Site or for educational activities, or (b) purchase audio conferences or other materials from us. Thus, your ability to use the Site may be limited. "Automatic information" is information automatically collected by our web server (which may be hosted by a third-party) that your web browser makes available whenever you visit our Site, such as your IP address and browser type. We do not associate this type of automatic information with personally identifiable information. We may collect information regarding your activities on the Site, including the parts of the website that you visit and the items you click on. This type of automatic information may be tied to your personally identifiable information, and will thus be treated as personally identifiable information. This Site uses "cookies." A cookie is a small text file that a website’s server places on a computer’s web browser. The cookie transmits information back to the website’s server about the browsing activities of the computer user on the Site. This may include information such as pages and content viewed and the time and duration of visits. Cookies also can be used to maintain data related to a particular individual, including passwords or items in an online shopping cart. Cookies do not alter your hard drive and cannot be used to access any personally identifiable information from your computer. Cookies are used as a standard practice by many sites. As important as cookies are to our Site, we understand that this might make you uncomfortable. Disclosure and Use of Information We Collect
We use personally identifiable information for the purposes for which it is provided. Thus, we may use your personally identifiable information to:
  • Establish your online account
  • Process and track your online registration(s)
  • Fill and confirm an order, and notify you of the order status
  • Invoice you or your organization, as applicable, receive and track your payments, or take follow up collection action
  • Provide the goods and/or services you request
  • Contact you about the educational activities for which you have registered
  • Track your completion of educational activities and your CME/CE contact hours
  • Issue accurate statements of completion
  • Respond to e-mails
  • Provide customer support
We may also use your personally identifiable information to communicate with you for marketing and promotional purposes, and we may provide it to any third party for that third party’s marketing or other commercial purposes. We may provide your personally identifiable information to third parties who provide services to us. These third parties may provide a variety of services to us including, without limitation, processing credit card transactions and sending e-mail communications to those on our e-mail distribution list on our behalf. If you prefer that your information be removed from our e-mail distribution list, you may unsubscribe at any time by selecting the "unsubscribe/change profile" link at the bottom of all Inquisit e-mails. If you participate in an activity for which Inquisit was awarded an educational grant, Inquisit may disclose your personally identifiable information to the entity that awarded the grant, and that entity may use such information for its own marketing purposes. We may disclose personally identifiable information (1) upon request from a CME/CE accrediting agency, in response to an annual audit, or (2) to companies assisting Inquisit in fraud protection or fraud investigations. We may disclose personally identifiable information to affiliated companies, our parent company and sister companies that either are subject to this Privacy Policy or that follow practices at least as protective as those described in this Privacy Policy. We use automatic information (including cookies and information regarding your browser type, operating system, and/or IP address) to customize the visitor’s experience, study traffic patterns, generally learn about the usage of the Site, and help us recognize visitors when they visit our site again. We may also use automatic information for the same purposes for which we use personally identifiable information, as detailed above. We may disclose automatic information to our affiliates and third parties, including, but not limited to, our advertisers. We may disclose personally identifiable information in special cases when we have reason to believe that disclosing this information is necessary to identify, contact, or bring legal action against someone who may have breached our Terms of Use or who may be causing injury to or interference with (either intentionally or unintentionally) our rights or property, other users of our Site, or anyone else who could be harmed by such activities. We may also disclose personally identifiable information when we reasonably believe that the law requires it. We might sell or buy assets, business units or companies, or participate in corporate mergers. In such transactions, personally identifiable information generally is one of the transferred business assets but remains subject to the promises made in any pre-existing Privacy Policy. Also, in the unlikely event that our assets are sold, personally identifiable information may be one of the transferred assets.
Privacy of Children
The Site is a general audience website intended for the continuing education needs of adult health care professionals. Our website is not intended for use by children under the age of thirteen, and we do not knowingly collect personally identifiable information from children under the age of thirteen.
Updating Your Information
If you would like to review and/or request changes to any of your personally identifiable information collected through the Site, you may do so by accessing your online account and using the account editing function available on the Site.
Security
We use reasonable efforts to protect your personally identifiable information from unauthorized use or disclosure. Unfortunately, information transmitted on the Internet and/or stored on systems attached to the Internet is not 100% secure. As a result, we do not ensure, warrant or guarantee the security or integrity of such information. We will not be responsible for disclosure of any information due to errors in transmission or the unauthorized acts of third parties. You help to maintain the security of your personally identifiable information stored in our systems by keeping your password confidential and protecting against unauthorized access to your password and to your computer. Be sure to sign off when finished using a shared computer.
