Best Practices: A Disciplinary Team Approach to Reduce Occurrences, Disabilities and Death Associated with Strokes
|Flower Hospital, an integral part of ProMedica Health System in Toledo Ohio, was voted the #1 most integrated health system in the United States by Modern Healthcare magazine in 2006 and 2008. In 2010 Flower Hospital was certified as a primary stroke center by The Joint Commission. This year they received the Stroke Bronze Quality Achievement award presented by AHA/ASA.
This program will be presented by some of the key members representing the Disciplinary Stroke team. They will share the steps taken to implement the Best Practice Stroke Program. Team goals and outcomes will be discussed as well as challenges and successes.
- Identify team members and their responsibilities.
- Discuss objectives for Stroke Center and performance improvement goals.
- Explain steps taken to reach goals including the Stroke Patient Flow.
- Describe the Challenges and Successes.
- Prepare for future plans.

Jeanne is Vice President of Patient Care Services for Flower Hospital where she is responsible for the development and implementation of strategic planning and goals for multiple hospitals. She and her staff recently achieved The Joint Commission Stroke Center Certification at Flower Hospital in 2010 along with the Bronze Award this year from AHA/ASA. Jeanne has had over 28 years of nursing experience in a variety of clinical areas with 14 of those years in management positions.

Marilyn for the last 3 years has been working on the Stroke Team under Special Projects at Flower Hospital. She has over 45 years experience in nursing with the most recent working in the department of neurology at Toledo Hospital and The Toledo Clinic. For close to twenty years she has worked at various facilities in Ohio, Colorado, Oregon and California as a staff nurse in intensive care.

Liz was recently promoted to coordinate the stroke program at Flower Hospital. Prior to this position she was Patient Care Supervisor of Pre-Admission testing for surgical patients. Liz has over 10 years experience in nurse management where her focus has been with the medical/surgical population.
